If you're a family member looking for residential care for a loved one, we're here to help you understand the process and find the right support.
Happy Life Group Homes provides care for adults (18+) with intellectual and developmental disabilities (I/DD), mental health challenges, and behavioral support needs. Our team is trained in behavioral interventions and crisis prevention, and we work closely with behavior support consultants to create individualized plans. We are not an assisted living facility for seniors or individuals whose primary needs are related to aging or dementia without a developmental disability diagnosis.
To be placed in one of our homes, your loved one must be enrolled in Oregon's I/DD services through your county's Community Developmental Disabilities Program (CDDP). Here's how the process works:
1. Contact your county CDDP
If your family member isn't already enrolled in services, contact your county's CDDP office to apply for eligibility. They will assess your loved one's needs and assign a case manager.
2. Work with the case manager
Once enrolled, a case manager will help develop a support plan (ISP) and can send referrals to residential providers like us. The case manager coordinates funding, services, and placement.
3. We receive the referral
When we receive a referral from the case manager, we review the individual's needs, schedule a meet-and-greet, and determine if one of our homes is a good fit. If so, we work together on transition planning.
If your family member's primary needs are related to aging, dementia, or physical disability without a developmental disability diagnosis, our homes may not be the right fit. We recommend calling 211 (or 1-866-698-6155) for help finding appropriate resources, including assisted living facilities, memory care, or in-home support.
We're happy to answer general questions and point you in the right direction. Call us at (503) 509-3703 or email info@happylifellc.org.
